In this vital role, you will deliver expert physiotherapy care to infants and toddlers, supporting their growth, development, and overall well-being. Your expertise will help families navigate early childhood challenges by creating individualized family service plans, and empowering caregivers with the tools they need. This position offers an exciting opportunity to make a meaningful impact during the most formative years of a child's life while working in a collaborative, supportive environment. Responsibilities Conduct comprehensive assessments of infants and toddlers to evaluate their physical development and identify therapy needs Collaborate with multi-disciplinary early intervention team, and medical professionals to optimize patient outcomes. Provide parent training for families to carry over strategies practiced during sessions in home and community settings utilizing natural learning environment practices. Monitor progress regularly, adjusting treatment strategies to optimize outcomes in inpatient, outpatient, home health, or clinic environments Assist with transition planning by coordinating with families, caregivers, and multidisciplinary teams to ensure seamless transition from early intervention services. Maintain detailed documentation in compliance with birth to three system procedures and personnel standards. Participate in interdisciplinary team meetings to review patient progress and modify care plans as needed for optimal results. Ensure compliance with safety standards in all therapy environments. Skills Strong knowledge of pediatric physiology and anatomy relevant to early childhood development Experience in discharge planning and creating effective care plans for young children with diverse needs Ability to utilize EMR systems efficiently for documentation and communication purposes Employee will use their own vehicle to drive to and from visits in the home/community and to the office locations, as needed. Office locations include primary Birth to Three Office or regionally-based offices for meetings and trainings.
