The PT evaluates, develops, and implements treatment plans designed to restore function, improve mobility, relieve pain, prevent disability, and promote overall health and wellness. The Physical Therapist works collaboratively with physicians, nurses, therapists, home health aides, and caregivers to achieve optimal patient outcomes while maintaining compliance with Medicare Conditions of Participation, AHCA regulations, and agency policies. Essential Duties and ResponsibilitiesPatient Assessment and Plan of Care Perform comprehensive initial and ongoing assessments of patients' physical therapy needs. Develop, implement, and revise individualized treatment plans based on physician orders and clinical findings. Establish measurable goals and expected outcomes in collaboration with the patient, caregiver, and interdisciplinary team. Regularly evaluate patient progress and modify interventions as needed. Obtain and communicate physician orders, recertifications, and updates to the plan of care. Skilled Physical Therapy Services Provide therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities. Assess and recommend adaptive equipment, assistive devices, and home safety modifications. Implement evidence-based interventions to improve strength, endurance, range of motion, coordination, and functional independence. Address pain management, fall prevention, and injury prevention strategies. Promote safe performance of activities of daily living. Care Coordination Collaborate with physicians, nurses, therapists, home health aides, and caregivers to ensure coordinated patient care. Communicate significant changes in patient condition promptly to the physician and clinical management team. Participate in interdisciplinary case conferences and care planning activities. Serve as a clinical resource to staff and caregivers regarding physical therapy needs and interventions. Supervise Physical Therapist Assistants (PTAs) and paraprofessional staff in accordance with state and federal regulations. Patient and Caregiver Education Educate patients and caregivers regarding diagnosis, treatment plans, home exercise programs, safety measures, and disease management. Provide instruction on the proper use of adaptive equipment and assistive devices. Reinforce strategies that promote independence and reduce the risk of hospitalization or injury. Participate in orientation and training activities for staff as assigned. Documentation and Compliance Complete all clinical documentation accurately, thoroughly, and within agency-required timeframes. Document evaluations, reassessments, treatments, supervisory visits, progress notes, and discharge summaries. Ensure compliance with Medicare, AHCA, HIPAA, and agency documentation standards. Accurately transcribe and implement physician verbal orders when applicable. Maintain timely communication with the office regarding schedule changes, patient concerns, and clinical updates. Professional Responsibilities Maintain the highest standards of professional ethics and conduct in accordance with the American Physical Therapy Association (APTA). Protect patient confidentiality and privacy at all times. Participate in quality improvement activities, meetings, and required in-service education. Maintain current licensure, certifications, and required competencies. Perform other duties as assigned to support patient care and agency operations. QualificationsRequired Graduate of an accredited Physical Therapy program. Current and unrestricted Florida Physical Therapist license. Minimum one (1) year of clinical experience as a Physical Therapist preferred. Current CPR certification. Valid driver's license, reliable transportation, and proof of automobile insurance. Strong assessment, communication, and clinical decision-making skills. Ability to work independently in a home health setting. AHCA Requirements Meets all Florida Agency for Health Care Administration (AHCA) requirements for home health personnel. Successful completion of Level II Background Screening. Current health records and TB screening as required by state regulations. Compliance with all Medicare Conditions of Participation and home health regulations. Completion of all mandatory agency orientation, in-service training, and continuing education requirements.
