Job Description

Position Details:

Location:
In-home patient evaluations Hiring for Naples, Ft.
Myers, Cape Coral, Lehigh Responsibilities:
  • Perform 5-7 patient evaluations per day
  • Each evaluation is approximately 30-40 minutes
  • Complete documentation in a timely and accurate manner
Ideal Candidate:
  • Licensed Physical Therapist (active state license)
  • Comfortable working independently in a mobile setting
  • Strong clinical and communication skills This is an excellent opportunity for a PT looking for flexibility, consistent volume, and efficient scheduling.
Responsibilities Conduct comprehensive evaluations of patients to assess their physical abilities and limitations. Develop individualized treatment plans based on patient assessments and goals. Implement therapeutic exercises and techniques, including trigger point therapy, to promote recovery. Collaborate with healthcare professionals in acute care settings to ensure coordinated patient care. Provide education to patients and their families regarding discharge planning and home community care options. Document patient progress accurately and maintain up-to-date records in compliance with healthcare regulations. Work with geriatric patients to address age-related physical challenges effectively. Participate in ongoing professional development to stay current with best practices in physical therapy. Requirements Degree in Physical Therapy from an accredited program. Valid state licensure as a Physical Therapist. Strong knowledge of human physiology and anatomy relevant to rehabilitation practices. Experience in hospital or acute care settings is preferred but not required. Familiarity with discharge planning processes and home health services is advantageous. Excellent communication skills for effective interaction with patients, families, and interdisciplinary teams. Ability to work collaboratively within a healthcare team while demonstrating strong patient care skills. Commitment to providing compassionate care tailored to individual patient needs.
