Responsibilities Conduct comprehensive patient evaluations to determine physical therapy needs using physiology and anatomy knowledge. Develop and implement tailored care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy and orthopedics. Assist patients in achieving their rehabilitation goals through active therapy sessions focused on mobility, strength, and pain management. Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient care across inpatient, outpatient, home health, and community settings. Educate patients and caregivers on injury prevention, home exercise programs, and community resources to support ongoing recovery. Maintain accurate documentation of treatment progress within electronic medical record (EMR) systems while adhering to healthcare regulations. Stay current with medical terminology updates and advances in physiotherapy practices to continually enhance patient outcomes. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience in acute care, hospital environments, or outpatient clinics demonstrating strong patient care skills. Knowledge of physiology, anatomy, and medical terminology essential for effective treatment planning. Experience with discharge planning, care plans development, and home & community care coordination. Familiarity with EMR systems for documentation and patient record management. Background in pediatrics, geriatrics, orthopedics, or home health is highly desirable. Strong communication skills coupled with the ability to work collaboratively within multidisciplinary teams.
