Responsibilities Conduct comprehensive patient assessments to determine physical limitations and develop personalized care plans Monitor patient progress regularly and adjust treatment strategies to optimize outcomes Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings Educate patients and their families on injury prevention, mobility exercises, and self-care strategies Requirements Valid licensure or certification as a licensed Physical Therapist in FL. Proven ability to develop effective care plans tailored to individual patient needs Excellent communication skills to educate patients effectively and collaborate with healthcare teams

Pay:

$45.00 - $55.00 per hour
