Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in patient care, particularly in orthopedics and acute care settings. This role involves developing and implementing personalized care plans, conducting assessments, and providing therapeutic interventions to help patients achieve optimal recovery and mobility. The Physical Therapist will work closely with patients in both inpatient and home community care environments, ensuring a smooth transition through discharge planning. Responsibilities Conduct thorough evaluations of patients' physical abilities and limitations. Develop individualized treatment plans based on patient assessments and goals. Implement therapeutic exercises and interventions to improve strength, flexibility, and mobility. Educate patients and their families about treatment options, home exercises, and lifestyle modifications. Collaborate with healthcare professionals to coordinate comprehensive patient care. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Maintain accurate documentation of patient records, treatment sessions, and progress notes. Participate in discharge planning to facilitate successful transitions from inpatient care to home or outpatient therapy. Requirements Degree in Physical Therapy from an accredited program. Valid state licensure or eligibility for licensure as a Physical Therapist. Strong knowledge of anatomy and physical therapy principles. Experience in orthopedics, acute care, or related fields is preferred. Ability to develop effective care plans tailored to individual patient needs. Excellent communication skills for interacting with patients, families, and healthcare teams. Familiarity with discharge planning processes and home community care systems is advantageous.
