We're now spreading the love through the country by opening our second location in Southwest Florida! We're looking for a PT who has experience in serving patients on a home health setting and is willing to serve potentail patients in the following areas and thier surrounding towns: Fort Myers, Lehigh Acres, Cape Corial, Bokeelia, and Harlem Heights ! This is a PRN opportunity, meaning your schedule depends on your availability. Even if you can only serve one of these areas, feel free to apply!
JOB DESCRIPTION
The ideal candidate for this role will be responsible for providing skilled Physical Therapy services on an intermittent basis to patients in their homes in accordance with physician orders Duties Provide initial, and ongoing, comprehensive assessments of the patient's needs Prepare, and present, patient's record to the Clinical Record Review Committee, as indicated. Prepare clinical and progress notes Assist in development, and implementation, of the interdisciplinary patient care plan, as it pertains to Physical Therapy. Assist the physician in evaluating patients, by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests. Supervise the Home Health Aide, every two weeks, when physical therapy is the only professional discipline involved in the patient's care and supervise the Physical Therapy Assistant, at least monthly. Performs other duties as assigned. Qualifications Graduated from an accredited Physical Therapy Must be licensed in the state of Florida as a PT Must be willing to travel to Immokalee One or more years in Home Health Agency or in a hospital setting. Home health experience preferred. Must have knowledge of Medicare and Medicaid guidelines. Must have working knowledge of or interest in home healthcare, the principles and techniques of professional nursing, and required documentation that pertains to it. Valid driver's license, dependable transportation, and valid automobile insurance Proficient with Microsoft Office products (Outlook, Word, Excel, PowerPoint) Must have a criminal background check. Must have current CPR certification. Excellent communication skills, both verbal and written, to effectively convey information. Highly organized with the ability to manage multiple tasks simultaneously.
