Job Description

Physical Therapist (PT) - Home Health Full-Time | Lee County, FL (Including Bonita Springs & Estero) A leading home health provider in Southwest Florida is seeking a compassionate and patient-focused Physical Therapist (PT) to join its growing clinical team in Lee County. This is an opportunity to provide high-quality, one-on-one care directly in patients' homes while helping individuals regain mobility, independence, and confidence in their daily lives. This organization is committed to delivering a concierge-level standard of home health care with a strong emphasis on empathy, reliability, quality, and clinical excellence. Clinicians are supported by a collaborative interdisciplinary team that prioritizes both exceptional patient outcomes and a positive work environment for its staff. Responsibilities Perform initial evaluations, reassessments, and OASIS documentation for home health patients Develop and implement individualized treatment plans in accordance with physician orders Provide skilled physical therapy services in patients' homes throughout Lee County Monitor patient progress and adjust treatment plans as clinically appropriate Communicate patient status updates, treatment responses, and care recommendations to physicians and interdisciplinary team members Educate patients and caregivers on exercises, mobility techniques, fall prevention, and use of assistive devices Assist with discharge planning and completion of discharge summaries Supervise Physical Therapy Assistants (PTAs) and Home Health Aides in accordance with agency and state regulations Participate in interdisciplinary care planning, case conferences, and team collaboration Ensure timely, accurate, and compliant clinical documentation Coordinate additional services and community resources as needed to support patient care Qualifications Graduate of an accredited Physical Therapy program approved by CAPTE or equivalent Active Florida Physical Therapist license in good standing Minimum of 1 year of Physical Therapy experience preferred; home health experience strongly preferred Experience with OASIS documentation preferred Current CPR certification required Strong clinical assessment, communication, and patient education skills Ability to work independently and manage a mobile patient caseload efficiently Valid driver's license, reliable transportation, and current auto insurance required Must successfully pass background screening and drug screening requirements Physical Requirements Ability to travel throughout Lee County, including Bonita Springs and Estero Ability to access patient homes that may have limited accessibility Ability to lift, transfer, and assist patients as needed Ability to bend, stoop, kneel, and perform physical patient care duties throughout the workday Strong verbal and written communication skills in English required Why Join Us?
