Jacksonville, FL Salary:
$35- 40/hour (based on yrs/exp) Evaluate and treat various orthopedic (spine/extremities) and neurological conditions, formulate treatment plan, establish patient goals, use good judgement and clinical reasoning/decision making throughout patient episode of care. 1-3 years of experience preferred. Therapist-owned Outpatient Private Practice

- PT will work with patients to restore function, improve mobility, decrease pain and improve overall health and quality of life for patients under their care.
- Working in an Outpatient Physical Therapy office in collaboration with Physical Therapy Assistants, treating patients with Orthopedic diagnoses, Neurological Dysfunctions and Fall Risk patients.
- Proper application of treatment modalities including HP/CP, ESTIM, US, massage and appropriate manual therapies within the scope of care of a PT. Appropriate progression of therapeutic exercise/activities, ADL training, gait training with patient population and caseload.
- Document all therapy interventions from evaluation through patient discharge.
- Web PT or other EMR experience preferred.
- All patient care documentation meets current professional and clinical standards. This includes daily progress notes, and updated patient POC/ prescriptions. Notes must be thorough/ complete, timely, legible and efficient.
- Communicate professionally with staff and patients.
- Attends required continuing education courses according to State of Florida regulations.
- Consistently meet patient care productivity standards for position.
- Demonstrate a proactive attitude toward patient compliance, assisting with patient scheduling, calling patients that cancel or no-show, and overall management of patient caseload to reduce patient attrition and increase therapy appointment attendance.
- Adhere to all Federal, State, and company billing rules and regulations.
