Physical Therapist (PT) Creekstone Health and Wellness Kissimmee, FL Per diem Per diem Physical Therapist (PT) Position Summary The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized physical therapy treatment programs for residents in a Skilled Nursing Facility (SNF). The PT works collaboratively with an interdisciplinary team to improve residents' mobility, functional independence, safety, pain management, and overall quality of life while complying with all federal, state, and facility regulations. Physical therapists in SNFs commonly treat geriatric patients recovering from surgery, illness, injury, stroke, or other medical conditions requiring rehabilitation.

Assess resident mobility, strength, balance, gait, range of motion, pain levels, and functional abilities. Provide skilled therapeutic interventions, including therapeutic exercise, gait training, balance training, transfer training, neuromuscular re-education, and manual therapy. Establish realistic short-term and long-term rehabilitation goals. Monitor resident progress and modify treatment plans as appropriate. Educate residents, families, and caregivers regarding treatment plans, home exercise programs, fall prevention, and safe mobility techniques. Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate resident care. Complete accurate and timely documentation in accordance with Medicare, Medicaid, Florida regulations, and facility policies. Participate in care plan meetings and interdisciplinary team conferences. Ensure compliance with all infection control, safety, and quality assurance standards. Supervise Physical Therapist Assistants (PTAs) and rehabilitation aides as permitted by state regulations. Maintain productivity standards and support facility performance goals. Qualifications Education Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program. Licensure Current Florida Physical Therapist License in good standing. Current CPR/BLS certification preferred. Florida licensure is required for practicing PTs within the state. Experience Skilled Nursing Facility (SNF), post-acute care, rehabilitation, or geriatric experience preferred. Experience treating orthopedic, neurological, and medically complex patients preferred. Knowledge, Skills, and Abilities Strong understanding of physical therapy principles and rehabilitation techniques. Knowledge of Medicare Part A, Part B, PDPM, and skilled nursing documentation requirements. Ability to assess, plan, and implement effective treatment programs. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Proficiency with electronic medical records (EMR) systems and therapy documentation software. Ability to work effectively within an interdisciplinary healthcare team. Physical Requirements Ability to stand, walk, bend, lift, transfer, and assist residents throughout the workday. Ability to lift and/or move up to 50 pounds with or without assistance. Ability to perform repetitive motions and maintain prolonged periods of standing and walking.
