Job Description Help for Job Description. Opens a new window. A Travel Therapist (PT) role is now available through Adecco Healthcare and Life Science. In this role, you will provide patient care in a home health setting across multiple communities, including developing individualized plans of care to be followed by the patient, caregiver, and Physical Therapist Assistant (PTA) during subsequent visits. You will collaborate with physicians to address patient needs and ensure appropriate care and actively participate in weekly interdisciplinary case conferences to support recertification and discharge planning. Additional responsibilities include all aspects of home health physical therapy, ensuring high-quality, patient-centered care.

Job duration: Minimum 3-month temp/contract role (potential for extension)

Monday-Friday 8:00am-4:30pm

$54

    On-Call/Weekend Requirement:
    None (flexibility available for schedule adjustments as needed) Key Responsibilities
    • Perform comprehensive patient evaluations and develop individualized treatment plans
    • Provide skilled physical therapy services in accordance with physician orders and care plans
    • Monitor patient progress and adjust treatment plans as needed
    • Complete accurate and timely documentation, including OASIS assessments
    • Collaborate with interdisciplinary care teams to ensure coordinated patient care
    • Educate patients and caregivers on treatment plans, exercises, and safety measures
    • Maintain compliance with all regulatory and clinical standards
