  • Job Summary
  • The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.
  • Essential Functions
  • + Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans.
+ Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented. + Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs. + Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information. + Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery. + Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff. + Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care. + Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs. + Maintains compliance with facility policies, including safety, infection control, and risk management procedures. + Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care. + Performs other duties as assigned. + Maintains regular and reliable attendance. + Complies with all policies and standards.
  • Qualifications
  • + Doctorate in Physical Therapy (DPT) required + 0-2 years of experience in a physical therapy setting required
  • Knowledge, Skills and Abilities
  • + Strong knowledge of physical therapy principles, therapeutic techniques, and safety standards.
+ Ability to perform comprehensive evaluations and develop effective treatment plans. + Proficiency in using therapy equipment and technology for documentation and billing. + Excellent communication and interpersonal skills to collaborate with patients, families, and interdisciplinary teams. + Strong organizational and time management skills to prioritize tasks and maximize productivity. + Commitment to professional development and adherence to ethical and legal standards.

  • + PT
  • Physical Therapist license in the state of employment required + BCLS
  • Basic Life Support within 30 days of hire required
