Job Summary Radiant Nursing and Rehab in Palatka, FL is seeking a dedicated and versatile Physical Therapist to join our healthcare team on a PRN (as-needed) basis. The ideal candidate will possess a strong foundation in physiotherapy, with expertise in developing and implementing individualized care plans to promote patient recovery and mobility. The Physical Therapist will play a vital role in delivering high-quality patient care by applying their physiology knowledge, medical terminology expertise, and specialized skills such as trigger point therapy and orthopedics. Pay starts at $50/hr and increases based on experience level. Responsibilities Conduct comprehensive assessments of patients' physical conditions, including evaluating mobility, strength, and functional limitations. Develop and implement tailored treatment plans that align with patient goals and medical directives, incorporating discharge planning when appropriate. Administer physiotherapy interventions such as manual therapy, trigger point therapy, and therapeutic exercises to improve patient outcomes. Collaborate with interdisciplinary teams to coordinate care across inpatient, outpatient, home health, and community settings. Educate patients and their families on treatment procedures, injury prevention strategies, and home exercise programs to support ongoing recovery. Maintain accurate documentation of patient progress within electronic medical record (EMR) systems while ensuring compliance with healthcare regulations. Stay current with advancements in physiotherapy practices, anatomy knowledge, and medical terminology to enhance treatment efficacy. Participate in care planning meetings and discharge planning processes to facilitate seamless transitions between care environments. Current Physical Therapist License in Florida Experience Proven experience working in inpatient hospital settings, outpatient clinics, or home health environments. Demonstrated ability to manage care plans for diverse populations including pediatrics and geriatrics. Knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice. Experience utilizing EMR systems for documentation and patient record management. Prior involvement in discharge planning processes to ensure continuity of care post-treatment.
