Physical Therapist

Community Health and Rehabilitation Center is a 101 bed facility, with a state of the art therapy gym! We are looking for a PRN Licensed Physical Therapist, with at least 1 year of health care experience, preferably in a long term care setting Our ideal candidate would have a strong customer service personality and would be able to preform these essential job duties, that are not limited to the following

• Assess functional levels of capabilities of Residents
• Provide Physical Therapy Treatments as described in Residents Plan of Care
• Develop Goals which are measurable and realistic
• Thoroughly document treatment progress accurately and timely
• Preform overall supervision to PTAs and rehab techs to ensure quality services
• Completes required facility forms and documents to ensure compliance with state and federal regulations
• Assists with transporting residents to and from Therapy
• Provides train and education to family, residents and staff
