Full Job Description Job Summary:

We are seeking a compassionate and dedicated Physical and Occupational Therapist to join our growing home health team. The Therapist will provide skilled therapy services to patients in their homes, helping improve mobility, strength, balance, endurance, and overall functional independence.

Responsibilities:

Perform physical therapy evaluations and reassessments Develop individualized treatment plans based on patient needs Provide therapeutic exercises, gait training, transfer training, and balance activities Educate patients and caregivers on safety, fall prevention, and home exercise programs Document patient progress accurately and timely according to agency and Medicare guidelines Communicate effectively with physicians, nurses, and interdisciplinary team members Participate in case conferences and care coordination

Qualifications:

Active Florida Physical Therapist license Minimum of 1 year experience preferred in home health or related setting Current CPR certification Valid driver's license and reliable transportation Strong communication and documentation skills Knowledge of Medicare and home health regulations preferred

Pay:

$40.00 - $50.00 per hour
