  • Full-time As a Home Health Physical Therapist, you'll practice at the top of your license while delivering personalized, one-on-one care in the comfort of patients' homes.
As a Physical Therapist, you will: Perform and document comprehensive initial evaluations to identify patient needs and establish individualized treatment plans. Assess physical conditions and determine functional goals prior to intervention. Develop and implement customized physical therapy programs to improve or restore mobility and physical function. Evaluate treatment effectiveness and modify care plans to maximize patient outcomes. Educate patients and caregivers on treatment plans, exercises, and safety techniques. Apply infection control and universal precautions while delivering patient care. Ensure continuity of quality patient care through timely and accurate documentation. Communicate effectively with patients, families, interdisciplinary teams, and healthcare professionals. Support agency philosophy, mission, and clinical standards. Complete documentation within company-established guidelines. Requirements Graduate of an accredited Physical Therapy program Current and unrestricted Physical Therapist license Minimum 1 year of Home Health experience preferred, not required Current CPR certification Valid driver's license and reliable transportation Current auto insurance HomeCare HomeBase experience preferred Physical Demands Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods Ability to lift, carry, push, or pull up to 100 lbs.
