Job Description
A Patient's Choice Home Health is a privately owned agency serving patients in St. Lucie, Martin, Palm Beach, Okeechobee and Indian River counties. We are looking for Physical Therapists to provide in-home patient visits. If you meet the qualifications below, please apply now! We are looking for persons who: Have experience developing and implementing individualized care plans to improve and enhance the quality of life for patients. Always exercise courtesy whenever patients, family members and visitors are present. Demonstrate a positive attitude and professionalism on the job.
Desired Qualifications:
Active Florida PT License CPR Card- certificate must be from the American Red Cross, American Heart Association or American Safety and Health Institute Alzheimer's & Dementia Training Physical Therapists MUST have the following: Driver's License Social Security Card Proof of Authorization to work in the US (if not a US citizen) Reference Information x2 Level 2 Background Screening (we look this up for you)
- if you do not have a level 2 screening you can register Statement of Good Health (Physical)
- Dated within 6 months and stating "Free of Communicable Diseases" Professional Liability Insurance (or willingness to obtain it)
