Job Description

Looking for a change of scenery and a chance to help others. Provide in-home skilled physical therapy services as a traveling Physical Therapist in St. Petersburg, FL. Deliver patient evaluation, individualized plans of care, therapeutic interventions to improve mobility, strength, balance, and safety, and education for patients and caregivers to promote independence. Ensure accurate, timely, and compliant documentation supporting medical necessity and homebound status. Collaborate with physicians and interdisciplinary team members, communicate changes in condition, participate in care coordination, complete OASIS, adhere to agency policies, and supervise PTA services when applicable. Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Florida physical therapy license required to start the assignment. Home health experience required. Experience with HCHB electronic medical record required. OASIS knowledge required and experience completing OASIS assessments. BLS certification from AHA or ARC may be required.
Minimum education requirement:
Bachelor's degree. Preferred Qualifications Experience supervising PTA services preferred. Familiarity with home exercise program development and caregiver education preferred.
Assignment Details Shift hours:
8:00 AM - 4:30 PM.
Work schedule:
Monday through Friday.
Weekly hour guarantee:
40 hours. Mileage reimbursed between visits from first patient to last patient.
On-call/weekend requirement:
