• Full-time, PRN We are a skilled nursing facility located in Stuart Florida.

We are committed to providing excellent rehabilitation services to our residents, whether the need is short term rehab, skilled nursing care, long term or hospice care. We are now hiring a Physical Therapist to become part of our rehabilitative team. The therapist will provide evaluation and therapeutic services as needed by our patients in addition to the following: The successfully Hired individual will plan, develop, organize, implement and evaluate direct therapy in accordance with current rules regulations and guidelines that govern long term care facilities. Participate in discharge planning, implementation and development of residence care plans, residence assessments, etc. The therapist will also provide supervision and guidance to physical therapy assistants. Assist with educating family and staff to optimize patient outcomes and safety. Perform job duties in accordance with state and federal regulations and facility policies and procedures. Requirements:

Bachelor's degree required Certified FL license in Physical Therapy Excellent communication skills Great clinical, personal, documentation and implementation skills Skilled nursing facility (SNF) experience preferred Ability to lead and mentor PTA staff members and work collaboratively as part of a interdisciplinary team. This position requires a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse.
