Key Responsibilities:
Evaluate assigned patients and develop, implement, and modify individualized treatment plans aligned with each patient's needs, goals, and physician direction Delegate and provide direction for all discipline-specific therapy services to assigned patients Promote functional program designs enabling effective community reintegration Educate patients, families, students, coworkers, and yourself with a commitment to professional growth Complete timely and precise documentation consistent with regulatory guidelines and clinic policies Ensure care aligns with physician orders and insurance approvals when required Foster a positive learning environment for students by providing supervision and development opportunities Uphold and model patient and employee safety through clinic-specific safety standards and organizational performance improvement initiatives
Desired Qualifications:
Demonstrated experience in hospital-based physical therapy, preferably in an outpatient setting Strong clinical reasoning and dedication to patient-centered care Active PT license and compliance with all state-mandated education and standards Commitment to ethical practice and discipline-specific codes of conduct Effective team player with strong communication skills Eagerness for continuing education and professional development
