Job DescriptionSouthern Rehab is seeking a Physical Therapist to join our team in Fayetteville, GA. This full-time role involves providing exceptional care in an outpatient setting using the latest evidence-based techniques. We believe in collaborative work with other healthcare professionals and colleagues to ensure dynamic treatment for our clientele. Assess patient goals and limitations to develop a therapeutic plan of care and therapy timelineCarry out therapeutic interventions using the latest evidence-based techniques that appropriately address a patient's specific needs and concernsDocument initial evaluations, daily notes, progress notes, and discharge summaries using Raintree electronic medical recordsMaintain patient records in accordance with federal and state regulations and company policy to ensure excellent standard of careProvide regular updates to patients, physician's and other therapists involved in the patient's careCommunicate effectively with team of healthcare providers and colleagues to ensure an exceptional continuum of careEducate patients, families and caregivers on a home exercise program that complements therapy sessionsParticipate in continuing education and other professional development opportunities to remain knowledgeable about new and emerging therapies.
