Perform comprehensive patient evaluations to assess functional limitations, rehabilitation potential, and appropriateness for home health physical therapy services. Develop, implement, and regularly revise individualized Plans of Care (POCs) in collaboration with physicians, patients, caregivers, and interdisciplinary healthcare teams. Provide skilled therapeutic interventions for patients with neurological, neuromuscular, orthopedic, musculoskeletal, and mobility-related conditions. Complete OASIS assessments and maintain accurate, timely documentation utilizing HomeCare HomeBase (HCHB) in accordance with agency, regulatory, and payer requirements. Evaluate patient progress and modify treatment plans based on ongoing assessments, clinical outcomes, and physician recommendations. Educate patients and caregivers regarding therapeutic exercises, safety techniques, fall prevention strategies, disease management, and rehabilitation goals. Assess, recommend, and provide training on the safe use of durable medical equipment including walkers, canes, wheelchairs, braces, prosthetics, and adaptive devices. Collaborate closely with nursing, therapy, physicians, case management, and interdisciplinary teams to optimize patient outcomes and reduce hospital readmissions. Maintain compliance with infection control standards, patient safety protocols, Medicare guidelines, and home health regulatory requirements. Supervise and collaborate with Physical Therapy Assistants and other rehabilitation team members as appropriate.COMPENSATION & BENEFITS
