Job Summary Locations:

Birkwood Village, Fort Madison, Iowa and Montrose Health Center, Montrose Iowa We are seeking a dedicated, resident-centered Physical Therapist (PT) to join our therapy team. This role is ideal for a PT who values meaningful relationships, interdisciplinary collaboration, and helping older adults maintain mobility, independence, and quality of life. Role Overview The Physical Therapist provides skilled evaluation, treatment planning, and therapeutic interventions for residents in our long-term care and assisted living community. You'll work closely with our therapy team, nursing staff, and families to ensure each resident receives individualized, compassionate care.

Key Responsibilities:

• Assess patient conditions and develop personalized treatment plans
• Lead therapeutic exercises and monitor recovery progress
• Educate patients and caregivers on rehab techniques and safety
• Collaborate with healthcare teams and maintain clinical records
• Apply therapy modalities and ensure patient-centered care

Qualifications:
