Job Description

Physical Therapist (PT) - Subacute Rehabilitation Silver Oak Nursing & Rehabilitation Center 455 31st Street, Marion, IA 52303 Per Diem Opportunity Available About the Role Silver Oak Nursing & Rehabilitation Center is seeking a compassionate and motivated Physical Therapist (PT) to join our in-house subacute rehabilitation team on a per diem basis . In this role, you'll evaluate and treat residents to improve mobility, strength, balance, and overall functional independence in a skilled nursing and subacute rehab setting. You'll work closely with an interdisciplinary team to help residents safely return to their highest level of function and quality of life. Key Responsibilities Evaluate residents' physical therapy needs, including mobility, strength, balance, gait, and endurance Develop and implement individualized treatment plans per physician orders Provide skilled physical therapy services to short-term rehab and long-term care residents Complete evaluations, progress notes, and discharge summaries accurately and on time Educate residents, families, and caregivers on treatment goals, progress, and safe mobility techniques Collaborate with nursing, OT, SLP, physicians, and other interdisciplinary team members Participate in care plan meetings and interdisciplinary team conferences Maintain compliance with state, federal, and facility regulations Qualifications Doctorate or Master's degree in Physical Therapy from an accredited program Current Iowa PT license or eligibility Skilled nursing or subacute rehab experience preferred (new graduates welcome) Strong communication, documentation, and organizational skills Ability to work independently and collaboratively
Pay:
From $45.76 per hour
