Day Shift Description:
Full-Time - 40 hours weekly; Days - 8 hour shift; New Hampton Medical Center This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client's home/residence.
ESSENTIAL FUNCTIONS
Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting. Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable. Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician. Communicates with physician any requests to revise the plan of care. Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.
POSITION QUALIFICATIONS
