Job Description Physical Therapist•

Overview:

Assessing and re-evaluating the client's condition and needs as required OASIS Start of Care for therapy only cases Provide patient care according to patient's care plan and needs Teach the patient and/or family members about care and support skills Communicate regularly with Nurse Case Manger, Clinical Care Coordinators, Interdisciplinary team, and overseeing physician Accurately and thoroughly document patient care within company policy Supervision of PTA Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life

Qualifications:

Active Illinois PT license (in good standing) Valid Driver's License and reliable vehicle with current insurance Current CPR Certification Minimum two (2) years of clinical PT experience preferred Familiarity with Electronic Medical Records (EMR) software Comfortable with technology
