$60.00/hour Flexible PRN or Part-Time scheduling Opportunity to work across two established campuses In-house therapy program with strong interdisciplinary support Supportive, team-oriented environment Meaningful, resident-focused care model Opportunity to directly impact resident independence and quality of life Position Details PRN or Part-Time role Split between: Imboden Creek Senior Living & Rehabilitation Imboden Creek Gardens Decatur, IL Flexible scheduling based on facility needs Key Responsibilities Evaluate residents and develop individualized treatment plans Provide therapeutic exercises and interventions to improve mobility, strength, and function Educate residents and families on home exercise programs and safety strategies Collaborate with nursing and interdisciplinary team members Maintain accurate and timely documentation Support comprehensive, resident-centered care planning Qualifications Active Physical Therapist license in the State of Illinois (required) Strong clinical, communication, and organizational skills Ability to work independently and across multiple locations Skilled nursing or long-term care experience preferred (not required) Commitment to high-quality, patient-centered care Why Imboden Creek?
