Coverage Areas- Fox Valley / St. Charles territory (Around 20-mile radius travel from St. Charles)

Job Type:

Full-Time & PRN Opportunities Available. Approximately 25-28 visits per week for full time Therapist and Minimum of 10 visits per week for PRN/Part time/1099 arrangement. We can go up 20 visits for

PRN/1099.

Per visit Pay model Evaluations, Follow-up visits, Discharge visits an

SOC-DOC/OASIS

assessments. Training & Orientation- Paid orientation/training at $25/hour and IRS-standard travel reimbursement. Monthly incentive/bonus program for additional earnings.
