Physical Therapist (PT) - Premium Rate & Total Schedule Flexibility The Mission Looking to supplement your income while maintaining full control over your calendar? Serenity Estates of Freeport is seeking a flexible, compassionate Physical Therapist to join our elite, in-house therapy team on a PRN basis. Life Evaluation & Assessment:
Conduct comprehensive initial and ongoing evaluations to pinpoint residents' rehabilitation needs.
Individualized Care:
Develop and implement evidence-based treatment plans tailored to each resident's personal goals.
Therapeutic Intervention:
Administer advanced techniques and exercises to improve mobility, manage pain, and restore independence.
Track Outcomes:
Monitor and document resident progress, actively adjusting care plans for optimal recovery.
Maintain Compliance:
Keep accurate, timely clinical records in line with Illinois state and federal standards. What You Bring A valid, current Physical Therapist (PT) license in the state of Illinois.
