Job Description

Job Summary The Physical Therapist implements Physical Therapy treatments and programs to facilitate customer's maximum functional level of independence. Physical Therapy Services may include but are not limited to: evaluation of patients, muscle strengthening and balance exercises, gait, safety awareness and endurance training, application of physical agents, neuromuscular reeducation, postural control and sensorimotor training, pain management and peripheral/vertebral mobilizations. Job Responsibilities Evaluates patients for initiation, change or discharge of plan of care. Develops and implements treatment plan incorporating treatment methods and goals with patient/caregiver input. Completes relevant documentation and record keeping of stated services and consultation with other disciplines and services per regulatory guidelines. Designs, fabricates, fits and educates in use of assistive devices, special equipment, splints and orthotics. Selects or adapts appropriate programs and activities tailored to patients' needs consistent with their physical and mental ability, enabling them to reach their full potential, and return to prior level of function, working or adapted environment. Performs home/work site evaluation when appropriate with recommendations to enable patient to return to a safe home/work environment. Consults with other internal and external team members, disciplines, or services to provide plan of care and recommendation to progress patient's rehabilitation. Educates or instructs patients, family members, caregivers, staff or team members of skills and techniques required for patient to attain maximum functional outcome. Attends and participates in customer care, staff conferences and other meetings. Develops and implements training programs for other healthcare members on Physical Therapy objectives and techniques. Prepares, cleans and ensures proper working order of patient care equipment. Reports repair needs to appropriate department per policy. Supervises support personnel, in accordance with state and professional standards. Adheres to FHN policies, procedures and PRIDE Standards. Displays competency for age specific patients: pediatrics to geriatrics (where applicable). Participates in process improvement/quality indicator tracking when applicable. Displays competency for the Physical Therapist role as evidenced in the annual performance appraisal following the
FHN PRIDE
Standards. Supervises Physical Therapist Assistants, Rehabilitation Technicians, Students, and Volunteers. Other duties may be assigned.
Requirements Education:
Doctorate or Master's Degree in Physical Therapy preferred; Bachelor's Degree from four-year College or University in Physical Therapy will be considered with relevant experience
Licensure/Certification/Registration:
Current licensure or eligible as a PT under the Illinois Physical Therapy Act Maintain AED/BLS certification
