Home Health Physical Therapist

Job Summary We are seeking a dedicated and skilled Home Health Physical Therapist to provide comprehensive rehabilitative care to patients in their homes and community settings. The ideal candidate will possess a strong foundation in physiotherapy, with expertise in developing personalized care plans that promote recovery, mobility, and independence. Responsibilities Assess patients' physical conditions through detailed evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies. Develop and implement individualized care plans that incorporate therapeutic exercises, trigger point therapy, and other physiotherapy techniques tailored to each patient's needs. Deliver hands-on therapy sessions focused on improving mobility, reducing pain, and restoring function across inpatient, outpatient, home health, and community settings. Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital or clinic to home or community care environments. Monitor patient progress regularly, adjusting treatment plans as necessary to optimize outcomes and facilitate recovery. Educate patients and caregivers on exercises, injury prevention strategies, and health management techniques to promote long-term wellness. Maintain accurate documentation within electronic medical record (EMR) systems in compliance with healthcare regulations and standards. Collaborate with healthcare professionals across disciplines to ensure comprehensive patient care that addresses physical, emotional, and social needs. Requirements Valid licensure as a Physical Therapist with relevant state or national licensing boards. Proven experience in physiotherapy practice within home health, inpatient hospital settings, outpatient clinics, or community care environments. Strong knowledge of physiology, anatomy, medical terminology, and orthopedic principles applicable to diverse patient populations. Experience with discharge planning processes and managing care for patients with complex medical conditions. Familiarity with EMR systems for documentation and patient record management. Hands-on expertise in trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient rehabilitation techniques. Ability to work independently while maintaining excellent communication skills within multidisciplinary teams.
