As a home care Physical Therapist, you'll provide exceptional one-on-one care to patients in the comfort of their own homes.

Minimum Qualifications:

• Education
• : Bachelor's degree with progressive experience or a Master's degree in Physical Therapy.
• Licensure
• : Must be licensed to practice as a Physical Therapist in Illinois.
• Experience
• : Previous experience in home healthcare or familiarity with conditions of participation is preferred.
• Additional
• : Must have a current driver's license and reliable transportation to travel to and from patient sites.
