Job Description
Provide treatment to assigned patients under supervision of Physical Therapist (PT) following established plan of care. Works collaboratively with supervising Physical Therapistother healthcare professionalsstaff to ensure patients reach maximum level of function. Essential Functions (Duties, TasksResponsibilities): Provide direct patient care following the Illinois State Practice Act (ISPA)department procedures following plan of care under supervision of PT. Communicate regularly with PT on patient progressany condition change. Provide patient &/or family educationhome exercise program instruction. Maintain patient records including progress notes, discharges, notes to physiciansrequired prescriptionssignatures. PTA
