In this role, you will establish individualized physical therapy goals, deliver high-quality treatments, regularly reassess patient progress, and maintain meticulous documentation to ensure optimal recovery outcomes.
Essential Functions & Responsibilities Patient Assessment :
Conduct thorough physical therapy evaluations to accurately assess patient needs and establish individualized treatment plans.
Therapeutic Interventions :
Administer safe, effective, and evidence-based therapy treatments, ensuring all interventions are appropriately documented.
Care Plan Optimization :
Reassess and adjust clinical treatment plans within appropriate timeframes to align with patient progress and changing medical needs.
Clinical Documentation :
Provide clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and medical billing information.
Patient & Family Education :
Educate patients and their families on treatment plans, therapeutic exercises, and post-discharge care to support long-term recovery.
Safety & Environment :
Operate specialized therapy equipment safely while maintaining a clean, organized, and secure environment for patients and staff.
Interdisciplinary Collaboration :
Partner seamlessly with referring physicians, peers, and interdisciplinary healthcare team members to ensure excellent continuity of care.
Demographic Competency:
Demonstrate population-appropriate competencies for all relevant patient demographics, tailoring clinical care to individual needs.
Compliance & Policy :
Maintain strict compliance with facility policies, including safety, infection control, and risk management procedures.
Professional Adaptability :
Adapt to clinical changes professionally and effectively, utilizing the proper chain of command to address and resolve issues impacting patient care.
Operational Standards :
Maintain regular, reliable attendance and comply with all healthcare policies, standards, and other duties as assigned.
Job Requirements & Qualifications Education & Experience Education:
Doctorate in Physical Therapy (DPT) from an accredited program is required
Experience:
0 to 2 years of experience in a physical therapy setting (New grads welcome to apply!)
Licenses and Certifications Licensure :
Active, unrestricted Physical Therapist (PT) license in the state of Indiana required
Certification :
Basic Life Support (BCLS) certification required within 30 days of hire Knowledge, Skills, and Abilities Strong foundational knowledge of physical therapy principles, therapeutic techniques, and clinical safety standards. Proven ability to perform comprehensive evaluations and develop goal-oriented treatment plans. Proficiency in utilizing therapy equipment and healthcare technology for electronic documentation and billing. Excellent communication and interpersonal skills to collaborate compassionately with patients, families, and healthcare teams. Strong organizational and time management skills to efficiently prioritize tasks and maximize patient productivity. A strong commitment to professional development, clinical excellence, and adherence to ethical and legal medical standards.
