Monday to Friday, 8:00 AM to 5:00 PM, rotating on-call weekends Position Overview As a Physical Therapist with Redcrest Home Health, you will deliver skilled, in-home care that helps patients regain independence and improve quality of life. You will collaborate with an interdisciplinary team and play a vital role in each patient's recovery journey. Compensation and Benefits Guaranteed base salary plus uncapped bonus structure Mileage reimbursement and additional incentive opportunities Medical, dental, and vision insurance options Paid time off, 401(k), and company-paid life insurance Employee Assistance Program and strong team support Qualifications Active Physical Therapist license in Indiana Minimum one year of acute care experience or home health experience preferred New grads are welcome to apply Strong clinical judgment and communication skills Valid driver's license and active auto insurance Key Responsibilities Provide in-home physical therapy services as ordered by the physician Evaluate patients and develop individualized plans of care Monitor progress and adjust treatment plans as needed Educate patients and families on exercises, safety, and recovery Complete accurate documentation in compliance with regulations Essential Functions Travel to patient homes within assigned territory Complete scheduled visits reliably and on time Document evaluations and visits within required timelines Follow all state, federal, and agency guidelines Deliver care within PT scope of practice Why Join Redcrest Home Health Work in a patient-centered, home-based environment Build meaningful relationships with patients and families Gain autonomy while supported by a collaborative team Contribute to improved outcomes and reduced hospitalizations Grow with a company that values clinical excellence
