Minimum Qualifications Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Indiana Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience required. Experience with HCHB (Homecare Homebase) required. Experience performing
SOC, ROC
evaluations, recertifications, and revisits required. Preferred Qualifications Previous travel therapy experience preferred. Experience working in St. Joseph and Marshall County home health markets preferred.

Expected hours/points:
27 points per week. Candidate must be able to work in a home health environment and complete documentation in HCHB.
