Home Health Physical Therapist 13 Week Contract $3000/wk Travel Stipend Available! Pay:
$3000/Wk plus .50 a mile for patient related travel Local and Travel Welcome! 13
Week Contract Schedule:
Monday-Friday 40 Hours per week
Service Area:
Perryton Must have Home Health experience Duties Assess patients' physical conditions and develop comprehensive care plans aligned with their goals and medical histories Provide hands-on physiotherapy treatments including trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient care Manage discharge planning processes to ensure smooth transitions from hospital to home or community care settings Document patient progress accurately using electronic medical record (EMR) systems and communicate updates with healthcare teams Educate patients and caregivers on exercises, injury prevention, and self-care techniques to promote long-term health Collaborate with multidisciplinary teams to coordinate patient care and optimize recovery outcomes Perform inpatient and home health visits across various settings such as hospitals, clinics, and community centers Requirements Valid licensure as a Physical Therapist with current state registration or certification Demonstrated experience in acute care, inpatient hospital environments, outpatient clinics, or home health settings Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy and orthopedics Proven ability to develop effective care plans for diverse populations including geriatrics and pediatrics Experience with discharge planning, patient care documentation, and EMR systems proficiency Familiarity with home & community care practices and outpatient therapy protocols Excellent communication skills to educate patients and collaborate with healthcare teams effectively Join us in delivering compassionate, expert physiotherapy services that transform lives!
