Position Summary We are seeking a motivated and compassionate Physical Therapist (PT) to join our dynamic outpatient rehabilitation team. The PT will evaluate, develop, and implement individualized treatment plans designed to improve mobility, relieve pain, and restore functional independence for a diverse patient population. Responsibilities Perform thorough evaluations to determine patients' functional status and treatment needs. Develop and implement customized treatment programs to achieve optimal outcomes. Provide manual therapy, therapeutic exercise, neuromuscular re-education, and patient education. Document evaluations, progress notes, and discharge summaries in accordance with clinic and payer requirements. Collaborate with other healthcare professionals and support staff to ensure coordinated, high-quality care. Maintain compliance with state practice laws, professional standards, and clinic policies. Qualifications Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program. Current State PT License (or eligibility for licensure).
