Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient transitions across care settings. Document patient progress accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations. Educate patients and caregivers on exercises, injury prevention strategies, and home care routines to promote ongoing recovery outside clinical settings. Requirements Valid certification as a Physical Therapist (PT) from an accredited program. Eligible for licensure with the Kansas State Board of Healing Arts. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques. Skilled in creating effective care plans tailored to individual needs across diverse populations including pediatrics and geriatrics. Proficiency with EMR systems for documentation and patient management. Experience in discharge planning, home & community care programs, and outpatient clinics.
