Job Summary:

As a Physical Therapist, you will provide in-home therapy services based on individualized treatment plans, collaborating with physicians and interdisciplinary teams to ensure optimal patient outcomes.

What you will do:

Complete initial patient evaluations within 48 hours of referral and conduct re-evaluations every 30 days to ensure appropriate plan of care. Develop, implement, and modify individualized treatment plans in collaboration with physicians and interdisciplinary team members. Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries. Provide education to patients and families on safe mobility techniques, proper use of adaptive equipment, and individualized home exercise programs. Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, high-quality care. Participate in case conferences, chart audits, and quality improvement initiatives to ensure compliance with regulatory standards and enhance patient outcomes. Qualifications Licensed Physical Therapist in Kansas . Graduate of an accredited PT program (APTA or AMA-approved).

Preferred:

2+ years of clinical experience, prior home health experience Valid driver's license and reliable transportation. Current CPR certification and compliance with required documentation.
