Physical Therapist

Physical Therapist This position is eligible for a $20,000 Sign-On Bonus The Physical Therapist assesses, plans, and implements specific treatment programs for individual patients and patient groups according to the principles and practices of physical therapy. Provides patient care/service based on identified needs/developmental levels for all patient populations.

Duties/Responsibilities:

Evaluates assigned patients within the guidelines established by the physician and in compliance with departmental policies and procedures. Implements competent treatment and performs regular reassessments for treatment modification, to maximize the patient s potential for functional independence. Provides patient/family/caregiver education based on the patient s disability, age, and psychosocial needs. Maintains accurate and up-to-date medical records and documentation timely.

Education:

Graduate of an accredited Physical Therapy program

Experience:

One year of clinical experience preferred

Licensure/Certification:

Valid Kentucky Board of Physical Therapy license Valid Kentucky driver's license (if home health)
