Duties Conduct comprehensive evaluations of patients' physical conditions using physiology and anatomy knowledge to identify impairments and functional limitations Develop and execute tailored care plans incorporating evidence-based physiotherapy techniques such as trigger point therapy and orthopedics interventions Assist patients in rehabilitation from injuries or surgeries by guiding therapeutic exercises and manual therapy techniques Manage discharge planning by coordinating with healthcare teams to ensure seamless transition from hospital to home or community care settings Document patient progress accurately using electronic medical record (EMR) systems and maintain detailed treatment records Educate patients and their families on injury prevention, mobility strategies, and home exercise programs to promote ongoing recovery Collaborate with multidisciplinary teams in inpatient, outpatient, hospital, or home health environments to optimize patient care outcomes Skills Strong understanding of medical terminology, physiology, and human anatomy to inform effective treatment strategies Proven experience in pediatric and outpatient settings Expertise in discharge planning, care plan development, and patient education for diverse populations including adult and pediatrics Proficiency with EMR systems for documentation and communication across healthcare teams Ability to assess and treat patients with various conditions, including geriatrics, pediatrics, orthopedics, or post-surgical rehabilitation Excellent communication skills to explain treatment plans clearly and motivate patients throughout their recovery journey.
