  • 50/hour Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility.
This role offers the opportunity to work closely with residents in both short-term rehab and long-term care while helping improve mobility, strength, balance, and functional independence.
Position Responsibilities:
Complete physical therapy evaluations and establish individualized treatment plans Provide therapy focused on gait training, transfers, balance, strengthening, endurance, and pain management Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident goals Monitor resident progress and adjust treatment plans as clinically appropriate Maintain timely and accurate documentation in accordance with facility standards
