Job Description

Fall Recovery & Prevention Program Lead - Occupational or Physical Therapist Our mobile outpatient therapy practice is seeking an experienced Occupational Therapist or Physical Therapist interested in leading a Fall Recovery & Prevention Program serving older adults in the community. Rather than traditional clinic-based roles focused on productivity targets, our practice emphasizes value-based, client-centered care delivered directly in the home and community environments where people live their lives. This position is designed for a clinician who enjoys autonomy, program development, and community education in addition to direct patient care. What makes this role unique The Fall Recovery & Mobility Program Lead will help shape how our practice supports individuals recovering from falls or experiencing declining balance and mobility. This clinician will serve as a clinical resource for fall-related care while also helping to educate families and community partners about preventing future injuries. Responsibilities Provide mobile outpatient therapy services to clients recovering from falls or experiencing balance and mobility decline Complete functional assessments including mobility, environmental safety, and daily activity performance Develop individualized treatment plans addressing strength, balance, coordination, and functional mobility Identify home safety risks and recommend practical environmental modifications Contribute to development of the practice's Fall Recovery Program protocols and educational materials Provide occasional community presentations at senior centers or caregiver groups related to fall recovery and safety Collaborate with physicians, case managers, and community partners to support safe aging in place Ideal candidate Licensed Occupational Therapist or Physical Therapist At least several years of clinical experience, ideally with older adults or neurological conditions Strong clinical reasoning and comfort working independently in community settings.
