Job Description

Physical Therapist (PT) - Join Our Team at Hadley Pointe for Nursing Rehab and Care! We are seeking a highly motivated and compassionate Physical Therapist (PT) to join our interdisciplinary team at Hadley Pointe for Nursing Rehab and Care in Hadley, Massachusetts. As a Physical Therapist, you will play a vital role in helping our patients achieve their rehabilitation goals and improve their quality of life. About the
Role:
As a Physical Therapist, you will provide high-quality physical therapy services to patients with various medical conditions, injuries, and disabilities. You will design and implement individualized treatment plans to improve movement, function, and overall well-being. You will work closely with our interdisciplinary team, including physicians, occupational therapists, and speech therapists, to ensure comprehensive care for our patients.
Responsibilities:
Conduct initial and ongoing evaluations to assess patients' physical function and identify treatment goals. Develop and implement comprehensive treatment plans, including exercises, modalities, and other interventions. Provide treatment sessions, including hands-on therapy and education for patients and their families. Progress patients' therapy programs, modifying treatment plans as needed to ensure optimal outcomes. Document patient progress and maintain accurate records. Collaborate with other healthcare professionals to develop and implement discharge plans.
Requirements:
Current licensure as a Physical Therapist in the state of Massachusetts. Bachelor's degree in Physical Therapy or related field. Experience working with diverse patient populations, including older adults, pediatric patients, and individuals with chronic conditions. Strong communication and interpersonal skills. Ability to work effectively in a fast-paced, dynamic environment.
