• Contract, Per diem Overview VCare HomeCare is seeking a dedicated Per Diem Physical Therapist to provide high‑quality, compassionate in‑home rehabilitative care to clients in the Holyoke area.

The PT will perform evaluations, develop individualized treatment plans, deliver evidence‑based interventions, and support clients in achieving optimal mobility, safety, and independence. Overview We are seeking a dedicated and skilled Physical Therapist (PT) to join our team on a per diem basis. The ideal candidate will possess a strong foundation in physiotherapy practices, with experience across inpatient, outpatient, hospital, and home health settings. This role involves delivering comprehensive patient care through personalized treatment plans, discharge planning, and collaboration with multidisciplinary teams to promote optimal recovery and mobility. Duties Conduct comprehensive initial and follow‑up evaluations of patient functional status. Develop and implement personalized treatment plans aligned with client goals and clinical best practices. Provide hands‑on therapeutic interventions to improve mobility, strength, balance, and functional independence. Educate paitnets and caregivers on home exercise programs, safety, and mobility strategies. Maintain accurate, timely clinical documentation in accordance with state and agency standards. Communicate effectively with nurses, care coordinators, and other interdisciplinary team members to ensure continuity of care. Adjust treatment plans based on client progress and changing needs. Experience Proven experience working as a Physical Therapist in inpatient hospital environments with exposure to acute care settings. Demonstrated ability to develop comprehensive care plans for diverse populations including geriatrics and pediatrics. Knowledge of physiology, anatomy, medical terminology, and various physiotherapy modalities such as trigger point therapy. Experience utilizing EMR systems for documentation and patient management. Background in home & community care or home health services is highly desirable. Familiarity with outpatient clinics or hospital-based physiotherapy programs is preferred. Strong understanding of discharge planning processes within hospital or outpatient contexts.
