Full Job Description Now Hiring:

OT / PT - Home Health (PRN) The PT (Physical Therapist) or OT (Occupational Therapist) will assess functional activity demands against the performance abilities of clients and identify deficits to improve quality of life and maintain or restore independence. The PT or OT is responsible for implementing standards of care for occupational or physical therapy services while ensuring all care is delivered in accordance with physician orders and established plans of care. This role is ideal for a dedicated PT or OT seeking flexibility and meaningful patient interaction. Schedule PRN - The PT or OT should be available for a minimum of 10-15 visits per week Responsibilities The PT or OT will evaluate client functional abilities and develop individualized therapy care plans The PT or OT will perform mobility, strength, balance, and functional movement assessments The PT or OT will implement therapy interventions based on physician orders and patient needs The PT or OT will provide therapeutic exercises, gait training, and functional activity training The PT or OT will educate patients and caregivers on exercises, safety, and strategies to maintain independence The PT or OT will monitor patient progress and adjust treatment plans as needed The PT or OT will maintain accurate and timely documentation, including electronic notes The PT or OT will ensure all care aligns with professional standards and best practices Qualifications Skilled PT or OT with experience in physical or occupational therapy Home care experience preferred but not required for the PT or OT Tech-savvy PT or OT preferred as documentation is transitioning to electronic systems Pay Up to $90 per visit (PT) Up to $85 per visit (OT) License / Certification Current PT (Physical Therapist) or OT (Occupational Therapist) license required CPR Certification required for all PT and OT candidates

$80.00 - $90.00 per hour
