(PT) Physical Therapist - Home Health BrightStar Care is dedicated to providing a higher standard of care that empowers clients to live well at every stage of life. Our professional care team, led by a Registered Nurse Director of Nursing, is committed to delivering expert, compassionate, and personalized care 24/7. Summary We are seeking a skilled (PT) Physical Therapist to join our team at BrightStar Care. The (PT) Physical Therapist will play a vital role in improving clients' mobility, independence, and overall quality of life through personalized rehabilitation plans. This position is ideal for a compassionate (PT) Physical Therapist who enjoys working directly with clients in a home care setting. Responsibilities The (PT) Physical Therapist will make home visits to clients in designated geographic territories The (PT) Physical Therapist will perform diagnostic tests and measurements including joint mobility, transfer ability, ambulation patterns, muscle strength, endurance, and balance testing The (PT) Physical Therapist will develop and implement individualized care plans with therapeutic exercises, gait training, and balance interventions The (PT) Physical Therapist will continually assess and revise the Physical Therapy care plan and collaborate with nursing staff on multidisciplinary care plans The (PT) Physical Therapist will educate clients, family members, and caregivers on rehabilitation techniques and safe mobility practices The (PT) Physical Therapist will conduct safety assessments within the home environment to help prevent injury and improve independence The (PT) Physical Therapist will accurately document observations, treatments, and evaluations related to client care Qualifications Current Maryland license as a (PT) Physical Therapist The (PT) Physical Therapist must be able to work independently and manage time effectively Strong interpersonal, organizational, and problem-solving skills expected from the (PT) Physical Therapist Solid computer skills; prior experience with electronic medical records (EMR) preferred for the (PT) Physical Therapist role This is an excellent opportunity for a dedicated (PT) Physical Therapist to make a meaningful impact while providing high-quality care to clients in their homes.
Pay:
$80.00 - $90.00 per hour
