Physical Therapist (13-Week Maternity Coverage)Join the Team at St. Andre Health Care! St. Andre Health Care is looking for a dynamic PT to join our interdisciplinary team for a 13-week per diem contract starting soon! The Role As our Physical Therapist, you'll be the heartbeat of our rehabilitation efforts. Conduct comprehensive evaluations to understand each resident's unique needs.Personalized Care:

This is a 13-week per diem contract specifically for maternity leave coverage.Physicality:
Ability to stand, walk, and bend frequently. You must be able to lift up to 50 lbs (utilizing a 2-person assist when required) to safely support our residents. Why St. Andre? Perform comprehensive resident evaluations per physician orders and establish therapeutic plans of care, including functional long- and short-term goals.Treatment & Progress:
Execute therapeutic procedures, monitor treatment effects, and continually progress the program toward established functional goals, modifying the plan as necessary.Collaboration & Communication:
Actively consult with physicians, caregivers, and the interdisciplinary team regarding the plan of care, discharge needs, and goal alignment.Documentation & Compliance:
Maintain accurate and timely documentation in the Electronic Medical Record (EMR) in compliance with department procedures and all reimbursement regulations (Medicare, Medicaid, etc.).Guidance & Mentorship:
Refer residents appropriately to Physical Therapist Assistants, ensuring collaborative care. Provide mentorship to new staff and in-service education to other departments.Assistive Devices:
Recommend and obtain appropriate assistive devices, ensuring residents and caregivers are fully instructed on their proper use.Required:
Bachelor's degree. (Master's or Doctorate degree preferred.)Mandatory:
Active license as a Physical Therapist (PT) in the applicable State of work.Preferred:
Basic Life Support (BLS) certification; prior experience in a long-term care or skilled nursing facility (SNF).Required Skills and Competencies Strong Communication:
Excellent verbal, written, and presentation skills (English proficiency required).Teamwork:
Supportive, responsible, and capable of working collaboratively in an interdisciplinary team setting.Clinical Acumen:
Expertise in patient care, vital signs monitoring, mobility support, and infection control.Organizational:
Excellent time management and detailed documentation skills.Technical:
Proficient in Microsoft Office. Physical Demands This role requires the physical and mental ability to perform tasks involving a combination of sitting, standing, walking, bending, stooping, twisting, and reaching.
