Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and/or instruction. Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being. Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client's age, condition, and needs. Implement client care plan activities based on treatment goals towards independent living for the client/family.What You'll Bring:
Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution. Current PT license in the State of Maine. Current CPR card required. 1-year Physical Therapy experience preferred. Home health experience preferred. A reliable car with insurance. Come see what we do with a ride-along or job shadow!
